The Past Employment Data (PED) Form should be completed by employers who have a new bargaining unit participating in the Trust for the first time. This document is your statement, as an employer, of each of your employee’s continuous date of hire. The data provided is used to determine Past Employment credits, if any, each employee is entitled to receive under the Plan by virtue of his/her history of employment with your company.
Once you download the form to your computer, you can fill it in, print it, sign and date it, and mail it back to an Area Administrative Office. (You cannot send the form via e-mail.)
